Scheduling an Appointment

  • You must schedule an appointment for the date and time you would like to visit our office to ensure your authentication request is completed in a timely manner. Schedule Now
  • If you’re unable to schedule an appointment, our office will have open walk-in services available Monday – Friday between 2 pm and 4 pm.
  • If you’re unable to schedule an appointment or come into the office during the open office period, you can fill out a request form and leave it in our office drop box. Requests filled out for the drop box are generally received and begin the process for authentication within one business day.

Fees and Processing Times

Regular Service Fee and Times

  • $20.00 per document (not per page)
  • 3 to 5 business days in office processing time (does not include mailing time).
  • A receipt will be emailed when the document is received (please provide an email address for this service).
  • Please allow additional days for documents to be mailed.
  • If picking up documents, we will notify you by phone or email when your documents are ready for pick up.

Next Business Day Service Fee

  • $55.00 per document.
  • Available for pick-up after 12 PM on the following business day.
  • A receipt will be emailed when the document is received.
  • Call or email for document status.

Same Day Service Fee

  • Suggested in office*
  • $95.00 per document.
  • Can take up to 2 hours to process, depending on availability and quantity of request.
  • A receipt will be emailed when the document is received.
  • *Due to late delivery times, documents mailed to our office with a request for same day service may not be mailed out the same day received. To ensure your document is sent back the same day we receive it you must include a prepaid return label waybill. (UPS, FedEx, DHL, USPS.)

International Shipping Fee

  • An additional fee of $10.00 will be charged for all International shipping (via USPS – No Tracking)
  • The Lt. Governor’s office is not responsible for items lost in the mail.
  • It is highly encouraged to provide a self-addressed prepaid envelope with a tracking number.

  • Documents are mailed back using State mail and USPS.  First class postage WITHOUT TRACKING will be affixed if envelope is not provided. Please note there is no ability to track the documents once they’ve left our office, and the Lt. Governor’s office is not responsible for any articles lost in the mail.

International Shipping Fee

  • An additional fee of $10.00 will be charged for all International shipping (via USPS – No Tracking)
  • The Lt. Governor’s Office is not responsible for any items lost using international shipping.
  • It is highly encouraged to provide a self-addressed stamped envelope with a tracking number.

Walk-in Information

  • We will call you when your documents are ready to be picked up.
  • If you choose to have your documents mailed back to you: documents are mailed using USPS first class postage. There is no tracking number, and the Lt. Governor’s office is not responsible for any articles lost in the mail.
  • There are no requirements for the document holder to either request or pick up documents for someone else.

Located in suite 220 in the Utah State Capitol Building, 350 N. State Street, Salt Lake City 84114. Parking available on the east side of the capitol hill complex.

The State Capitol building is the most northern point of State Street in Salt Lake City.

All document services require payment.

  • Checks made payable to “State of Utah”
  • Cash (please note sending cash in the mail is not advised)

Important Information

If you would like your documents mailed back to you, we highly encourage you to provide us with a self-addressed prepaid envelope with tracking. We do not provide tracking numbers and are not responsible for items lost in the mail.

*Please note that all mail sent through the United States Postal System may arrive later than intended, as all mail must be sorted and processed by the Utah State Mail Agency before arriving at our office. To ensure that your document arrives when you intend, please use the services of a commercial carrier such as FedEx. If you choose to use UPS to mail your documents back, please make sure to schedule a pickup with UPS as they do not visit our office every day.
