Here is a common list of Utah documents the Lt. Governor’s Office may authenticate:
- Original public documents that are certified by State of Utah agencies do not need notarization. These types of documents include (this is not a comprehensive list):
- Birth certificate – must be a certified copy from Vital Records
- Cremation/burial transit permit – must be an official copy from the Utah Department of Health
- Charter school document – must be a certified copy from the Utah charter school with the signature, name and capacity of the school official signing the document.
- Court record – must be a certified copy from the Court Clerk of the District Court
- Divorce decree – must be a certified copy from the Court Clerk of the District Court where the divorce is recorded
- Driver license record – must be a certified copy from the Utah Driver License Division
- Marriage license – must be a certified copy from the County Clerk’s Office or from Vital Records
- Public school document – must be a certified copy from the Utah public school with the signature, name and capacity of the school official signing the document.
- Single status document – must be an official, signed copy from a County Clerk’s office
- State background checks (BCI) – must be a certified copy of the background check with the signature of an official at the Utah Bureau of Criminal Identification. (Can also include a notarization, if requested from the BCI.)
- Vehicle title – must be an official copy from the Utah Division of Motor Vehicles
- Notarized documents. Documents properly notarized (see below) by a Utah notary may be authenticated. These documents include the following (this is not a comprehensive list):
- Adoption papers
- Authorization to conduct business in foreign countries
- Copies of passports, driver’s licenses/IDs, social security cards, etc.
- Corporate documents – must be a certified copy from the Department of Commerce and notarized at the Department. Emailed/printed copies from the Department will not be accepted.
- Deed of assignment
- Distributorship agreement
- Local police/sheriff office background check
- Police clearances
- Power of attorney
- Private school documents
- References, job certification, and employment letters
- School records – transcripts, report cards or diplomas (Can be notarized using a Copy Certification)
- Documents Not Authenticated by the Lt. Governor’s Office
- Documents notarized by a remote notary
- Public documents issued from outside of Utah (Including foreign countries)
- Documents that originate from a different state (Roster of Secretaries of State/Lieutenant Governors)
- Federal documents – must be sent to the US State Department
- Documents not notarized, if they are not public
- Documents that are improperly notarized – missing notarial language, stamp, and/or signature
- A document regarding allegiance to a government or sovereignty
- FBI background checks
